Annual checkups
A full head-to-toe review once a year, with bloodwork ordered in advance so results are ready when you sit down.
Primary care that follows you across decades, not appointments — with one named clinician who actually knows your file.
Family medicine is the front door of the practice. Roughly four in five patient questions are resolved here without ever needing a specialist — and when they do, your care lead makes the referral personally.
We keep panels deliberately small. A physician here carries fewer patients than the provincial average, which is the only reliable way to make continuity real rather than aspirational. It means when you call in March about something you mentioned in October, the person reading your chart remembers the conversation.
Continuity isn't a feature we added. It's the reason the clinic is organised the way it is.
Dr. Ilana Weiss — Medical DirectorEvery visit ends the same way: a written summary of what we discussed, what we decided, and what happens next. You leave with it in your hand or in your portal — never with a vague instruction to "come back if it gets worse."
A full head-to-toe review once a year, with bloodwork ordered in advance so results are ready when you sit down.
Structured follow-up for diabetes, hypertension, asthma and thyroid conditions on a recall schedule we track for you.
Childhood schedules, seasonal vaccines and travel immunisation, all recorded in one shared chart.
Periodic medication reconciliation to catch interactions and retire anything you no longer need.
First-line assessment and ongoing support, with warm handover to psychology when that's the right step.
We make the referral, chase the appointment and read the report back to you when it lands.
The same physician can follow a family through four decades. What we focus on shifts with each stage.
Forty-five minutes sounds generous until you realise how much of a medical history there is to cover properly. Here's how we spend it.
Book Your First VisitWe go through the records we requested beforehand, so you're not reciting your own history from memory.
A physical assessment appropriate to your age and concerns, including baseline vitals and bloodwork orders.
The part most clinics cut. We talk through findings, options and anything you came in worried about.
Your care lead is confirmed, follow-ups are booked, and the summary is printed or sent to your portal.
Nine physicians and four nurse practitioners rotate across our two sites. You'll be matched with one as your named lead.
Co-founder. Special interest in complex chronic disease and geriatric care.
Paediatric primary care and adolescent mental health. Speaks English and Yoruba.
Runs our chronic disease recall clinics and medication review sessions.
Anything not covered here, our reception team can answer during clinic hours.
Ask Our Team →Yes, intake is open at both sites. We keep panels small, so there are occasional short waitlists for a specific physician — reception will tell you honestly which leads have capacity this month.
That's the intent and it holds for most routine visits. For urgent same-day problems you may see whoever is on shift, but they'll be reading the same chart and your lead is notified afterwards.
Please do — that's why standard slots are 25 minutes. If you have several substantial concerns, mention it when booking and we'll allocate a longer slot rather than rushing you.
Results are reviewed by a clinician before release, then posted to your portal with a plain-language note. Anything abnormal generates a phone call rather than a notification.
Yes. Sign the release at intake and we request them directly. A records transfer fee may apply at the sending clinic's discretion, not ours.
